LOAN INTERVIEW INFORMATION

 Applies to Applicant and Co-applicant

 

Copy of signed Sales Contract on property to be purchased, along with any Amendments or Riders;

 

Social Security #'s, & valid Drivers License / State I.D. Card;

 

W-2 forms for the past 2 years; if self-employed, bring past 2 years signed tax returns & current profit/loss statement;

 

Most recent pay stub indicating year-to-date earnings;

 

Other income:               Source and monthly $ amount, & supporting documents such as divorce decree, lease, W-2;

 

Certificate of Eligibility and DD-214 (VA loans only);

 

$ Value of furniture & personal belongings;

 

If bankruptcy has been filed within the past 7 years, bring copy of bankruptcy petition, schedule, and discharge;

 

Releases of any judgments or liens, and letter of explanation;

 

Divorce decree, payment schedule, property settlement;

 

Residence address for the past 2 years;

 

Employment information for the past 2 consecutive years:

Name, address, and exact dates of employment;

 

List of all bank/credit union accounts:

Name and address of institution

Account #'s and type of accounts

Balance and most recent statement;

 

List of stocks, bonds, and other securities:

Name of stock, $ amount/share, # of shares & most recent brokerage statement;

 

                                                Insurance portfolio:        Employer insurance $ amount

                                                Other insurance - amounts of coverage & cash values;

                                                Vehicles:          Year, make, model, and $ value of all vehicles;

                                                Liabilities:         Complete list of names, addresses, account #'s, monthly

                                                                        payments and unpaid balances on bank cards and department

                                                                        store cards;

                                                Mortgage holder or landlord information (account #, unpaid balance, monthly                                           payment, equity - FHA, VA, CONV).
